Livewell Passport – Cancellation, Billing & Refund Policy
Cancellation Notice Requirement
All Live Well Passport memberships, subscription services, and recurring charges require a minimum of thirty (30) days’ written notice to cancel. It is the sole responsibility of the patient to initiate and confirm cancellation if services are no longer desired.
Ongoing Billing & Responsibility
Until a cancellation request has been properly submitted and the 30-day notice period has been completed, monthly billing will continue without interruption. Failure to cancel in accordance with this policy constitutes authorization for continued billing.
No Refund Policy
All charges are final and non-refundable once processed. This includes, but is not limited to:
Membership fees
Subscription or recurring charges
Services rendered or made available
Charges incurred during the 30-day cancellation notice period
Live Well Passport does not issue refunds, credits, or prorated adjustments for unused services, partial months, missed appointments, or failure to cancel.
Medical Services Disclaimer
Live Well Passport provides access to wellness-related medical services, consultations, and membership-based programs. Membership fees cover access and administrative availability, not guaranteed treatment outcomes or specific medical results.
HIPAA & Patient Responsibility
All patient information is handled in compliance with the Health Insurance Portability and Accountability Act (HIPAA). Cancellation of services does not affect Live Well Passport’s obligation to retain medical records as required by law. Patients remain responsible for any outstanding balances incurred prior to or during the cancellation notice period.
